Notification of Hospital Discharge Appeal Notices

CMS has two new notices and form instructions associated with CMS-4105-F: Notification of Hospital Discharge Appeal Rights that must be used beginning July 2, 2007.

Link to "An Important Message from Medicare About Your Rights" form (CMS-R-193) revised by eQHealth (includes instructions for its completion).
The notices are also posted at www.cms.gov/bni, click on “Hospital Discharge Appeal Notices.” Check this page frequently for updates.
