Notification of Hospital Discharge Appeal Notices

CMS has two new notices and form instructions associated with CMS-4105-F: Notification of Hospital Discharge Appeal Rights that must be used beginning July 2, 2007. 

Link to "Detailed Notice of Discharge" form (CMS 10066) revised by LHCR (includes instructions for its completion).

Link to "An Important Message from Medicare About Your Rights" form (CMS-R-193) revised by LHCR (includes instructions for its completion).
  

The notices are also posted at www.cms.hhs.gov/bni, click on “Hospital Discharge Appeal Notices.”  Check this page frequently for updates.
