Congratulations! to the below listed agencies for achieving improvement in ACH and/or their target outcome (most chose Improvement in Bathing).  The data period represented is October 2004 through August 2007.   

Since this is our final set of recognitions, we first want to extend a big THANK YOU to all agencies who worked so hard on improving their rates.  Please don’t get discouraged but keep up the great work and improvement will follow. 

Special Congratulations goes out to our Grand Award winners who achieved the most improvement in each performance group for ACH and Improvement in Bathing.  Information on award ceremony will follow as soon as plans are finalized.  The winners are:

Certificates with seals were awarded by performance group as follows:
Bronze status – relative improvement of 25% to 30%
Silver status – relative improvement of 30% to 35%
Gold status – relative improvement of 35% or higher
