Listed below are five sample POAs for the Acute Care Hospitalization and Bathing indicator.  Please feel free to download these POAs, evaluate them and make adjustments to suit your agency and plan. Then submit your plan to LHCR. The documents are Word files so you can easily modify the sample Plan of Action.
