  VI - Validation Appeal Process

A.   Appeal form located on Non-Secure QualityNet Home Page.  Select “Hospitals” from the Blue menu bar (across the top of the page), select “Data Validation” from the Hospitals drop down menu; scroll to bottom of page: click on “Data Validation Results Appeal Form, Part One”. 

B.   Save form to your computer and complete. 

C.   Log-In to QualityNet

D.   Send completed appeal form to LHCR via your QualityNet Secure “File Exchange and Search/Inbox Outbox” found under "File Exchange & Data Upload" (left column, 3rd Gray Box).  The appeal contains patient identifiable data!

E.   To upload form: click “File Upload Wizard”

F.   Select Organization: Louisiana Health Care Review

G.   Then click "View ALL Users" and add selected user(s).

H.  To attach the Appeal click: “Upload Files"

I.   Select the appeal form from your hard drive and the file will be sent.

q       Successfully uploaded (#) files” box will appear

J.   Please also notify your LHCR QI Specialist via their LHCR email address (see above) when you upload the completed appeal into the QualityNet Inbox. This will assure that QI Specialists who may be out in the field will be notified that your appeal has been sent.

þ    For additional information regarding data validation, go to the non-secure QualityNet Home Page.  Select “Hospitals” from the Blue menu bar (across the top of the page) and select “Data Validation” from the Hospitals drop down menu.

þ   IMPORTANT: Appeals must be submitted to LHCR within 10 business days after the validation results are posted on QualityNet This date is located on the Hospital Validation Overall Results Report)

þ    Please submit to LHCR no later than 5p.m. on the 10th business day to assure timely review and submission of the appeal to the CDAC.
